NEW PATIENT intake form

Cancellations made with less than 48 hours notice (2 business days) and no-shows will incur a $120 fee.

​We highly value you as a patient and trust that you will value us as your service provider. A canceled or late appointment delays our work and can negatively impact other patients. Since your appointments involve the reservation or time specifically for you, and out of respect for your therapist and our other patients, a minimum of 48 hours notice (2 business days) is required for rescheduling or canceling an appointment. Please call or text us at 206.604.4707 so that we have time to fill your slot and can put another patient in your space.

A late cancel or no-show appointment impacts at least three people: you, your therapist, and another patient who could have potentially utilized your time slot. Therapy sessions are scheduled in advance and are a time reserved exclusively for our patients. When a session is canceled without adequate notice, we are unable to fill this time slot by offering it to another current patient or patient on the waitlist.

• A credit card on file is required at time of service for your evaluation appointment
• Late arrivals of 15 minutes or more will result in the cancellation of your session and a no-show fee per above guidelines
• If you miss 2 appointments without notice, all future appointments will be canceled
